Diltiazem Ointment 2%

I have mentioned from time to time I would discuss how compounding is helping by a particular formulation or compound. Also, I will briefly discuss Pavilion Compounding’s method of formulating this ointment.

Today, for no apparent reason, I will look at a treatment option for anal fissures. What is an anal fissure? According to the American Society of Colon and Rectal Surgeons and I quote;

“An anal fissure is a small tear or cut in the skin that lines the anus.  Fissures typically cause pain and often bleed.  Fissures are quite common, but are often confused with other causes of pain and bleeding, such as hemorrhoids.”

Ok, so now that we have the definition, what do we do to help with this problem?

Usually the colo-rectal Md’s will use diltiazem ointment 2% as a treatment option to relax the sphincter muscle and allow the healing process to begin.  Another option are various strengths of nitroglycerin or nifedipine ointment, but both of these have more side effects, mostly headaches.  As if the fissure was not already causing a headache.  When compounding the 2% Diltiazem ointment, is is important to run the ointment thru an ointment mill to reduce the particle size and lessen the grittiness of the final product. This procedure also allows a larger amount of the drug to be absorbed into the rectal tissue.  Nothing worse than a gritty rectal ointment….so, ask your compounding pharmacist,one,  if they are using the pure diltiazem powder which makes for a finer product, and two, if they are taking the extra step to run the ointment thru the mill.  Using those two methods will ensure you are getting a superior compounded product. Smooth, well absorbed, and elegant.

What is Pharmacy Compounding?

We often get the question from a new patient, “What exactly is a compounding Pharmacy.”

And my other conversation begins as below.

“I never knew this was still being done, I seem to recall my grandfather talking about the pharmacist in the back room stirring something up that looked messy.”

Our premier compounding supplier, Professional Compounding Centers of America, (PCCA) has put together a short, informative video giving some insight into the specialized field of pharmacy compounding.

My other question is “don’t they teach this in pharmacy school?” Well yes and no, the pharmacy schools provide a very basic introduction to compounding, but the vast majority of the programs are to train pharmacists to work in community pharmacies and hospitals.  While the training is very relevant and intense, the specialty of compounding is like any other discipline of medicine, where our skills are taught beyond what is taught in pharmacy school.

Translated in plain english, a compounding pharmacist can provide options to your health that may not be apparent to your prescriber. Suppose you are looking to give a blood pressure medication to an infant, well we would work with your doctor to formulate this into a liquid.  Or maybe you need to give your cat an antifungal drug, how about a chicken flavored liquid designed in a concentrated form for ease of administration.  Or maybe you have knee pain and an stomach ulcer and you can’t take anti-inflammatory drugs by mouth. How about a topical gel designed to reduce your swelling and reduce pain thru the skin avoiding the stomach: Problem Solved!!!

Bottom line: we use our knowledge to help solve problems for you or your family members.  Ok my last question I get is “doesn’t my doctor know about this?”  Some do and some don’t. I often get calls from providers asking our help with a medication need.  Our job, as compounding pharmacists,is to help with alternatives, formulation expertise, and advising the doctor on safe, efficacious methods to customize that specific medication need.

Autologous Serum Eye Drops

Are you suffering from severe dry eyes?

If so, and you have failed to receive relief from over the counter eye lubricants or in the case of more severe problems even Restasis®, your own body may have the answer.

Artificial tears do not provide the essential nutrients necessary for healthy eyes. Autologous serum eye drops or “the healing soup” contains many components that aid in the healing and health of the eyes. Vitamin A, epithelial growth factors, insulin like growth factors, substance P, as well as other natural components all assist in providing superior eye lubrication and possible healing of damaged tissues.

Illnesses which can benefit from the serum eye drops include Sjorgren’s syndrome, Stevens-Johnson syndrome, which destroys the tear glands, chemotherapy, and various auto-immune syndromes.

Logistically, the procedure requires planning ahead for the blood draw, you can contact Pavilion Compounding Pharmacy in Atlanta for details on the options.  Basically, in simplistic terms, your own blood is drawn, spun down in a centrifuge, and the plasma is compounded under sterile conditions into the nourishing eye drops.  As this is a sterile procedure, the time to compound varies with the amount of blood drawn, as well as how many patients are being helped on any given day.  Allow at least 2 to 3 hours from the time of blood draw to the time you may pick up your drops.  There are several good restaurants in the Buckhead area, as well as shopping options that can be just a short trip while we are making the drops.

At Pavilion Compounding Pharmacy, we compound these as preservative-free eye drops, so keeping the drops cold in transport is important.  We provide cold bags, but it is encouraged, if you have a long distance to travel, which many of our patients have, to bring along a cooler to assure compliance with the storage requirements.

We have found many doctors and other health care providers are not familiar with this therapeutic option and we would be happy to discuss the benefits and procedures necessary for a smooth, convenient, procedure.

Bleaching compounds

A number of formulations are compounded to provide a customize approach to a bleaching skin care.  Most of the formulas contain hydroquinone, a bleaching agent, in concentrations ranging from 2.5% up to 12%.  Our Tri-hydro-Ret formulas also contain retinoic acid and triamcinolone or hydrocortisone as the steroid. Several of our doctors have the above compounded into a cream base, however; our formulas also have  a more occlusive option with an ointment.

Dr. Isaac Willis, used quite successfully, formulas that provided excellent results.  With Dr. Willis’ untimely passing, Pavilion Compounding Pharmacy was able to obtain his original formulations for his former patients.  Contact us for doctors familiar with Dr. Willis’ treatment options.
